All incoming students are required to complete: 1) DSPS application and 2) upload most recent disability related documents
***Documents must be unlocked and have a file extension of: pdf, doc, or docx***
Documentation Examples:
- An Individualized Education Plan (IEP) / 504 Plan
- A psycho-Educational Assessment from a School Psychologist
- Vision acuity results for low vision, blind students, and vision impaired
- An Audiogram (Deaf and Hard of Hearing Students)
- A Regional Center IPP
If you do not have any documentation but receive care from a professional provider, then please complete the Disability Verification Packet and the DSPS Application.
